Get started with forms.app's gym form templates or creates one for yourself. When you integrate your form with your Uplaunch account via Zapier, you can store your form data as a customer record in your UpLaunch account or create an appointment.

● UpLaunch account/workspace

● A Zapier account

How to set up this integration?

1. Click the 'Use This Integration' button.
2. When you are taken to Zapier's page, click the “forms.app + UpLaunch Connect” button.
3. Sign in to your Zapier and forms.app accounts.
4. Follow the simple steps to set up the integration you like.
5. End the integration and enable Zap.
