When you use forms.app as the trigger and Setmore Appointments as the action in Zapier, your workflow becomes: a submission in forms.app → Zap triggers → a new appointment or contact is created in Setmore Appointments with the form data. This ensures your business captures bookings and leads from the form automatically.

Setmore’s Zapier integration supports actions such as Create AppointmentNew Contact, and Update Contact.

What you’ll need

  • forms.app account (to build your form and collect responses)
     
  • Zapier account (to build the integration from forms.app → Setmore Appointments)
     
  • Setmore Appointments account (to receive the submission data as bookings or contacts)
     

5 Steps to Set Up Setmore Appointments + forms.app Integration
 

  1. In your forms.app dashboard, build and publish your form so that submissions will trigger the “New Form Submission” event via Zapier.
     
  2. In Zapier, click “Make a Zap” → choose forms.app as the trigger app → select the “New Form Submission” trigger and connect your forms.app account.
     
  3. Add the next step: select Setmore Appointments as the action app → choose an action such as “Create Appointment” or “Create Contact” (depending on whether you want to schedule or simply capture the lead) and connect your Setmore account.
     
  4. Map the form submission fields (for example: First Name, Last Name, Email, Phone, Date/Time, Service Type) into the corresponding Setmore fields (e.g., appointment start/finish, customer details) so the data lands correctly.
     
  5. Test your Zap with a sample form submission → verify in Setmore Appointments that the appointment or contact appears as expected → once confirmed, turn the Zap ON.
