When you use forms.app as the trigger and MySQL as the action in Zapier, your workflow becomes: a form submission in forms.app → Zap triggers → a new row (or update row) is created in your MySQL table with the form data. Zapier’s MySQL integration supports triggers like “New Row”, “New or Updated Row”, and actions like “Create Row”, “Update Row”.

What you’ll need

 

  • forms.app account (to build your form and collect responses)
     
  • Zapier account (to build the workflow)
     
  • MySQL database instance, accessible from the internet (so Zapier can connect)

 

5 Steps to Set Up MySQL + forms.app Integration

 

1. In your forms.app dashboard, create and publish your form so you begin collecting submissions.

2. Go to the connect tab when creating your form, or use the button on this page to go to MySQL + forms.app workflow in Zapier.

3. In Zapier, select “Create Row” (or “Update Row” if you need to update existing records) and connect your MySQL database (you’ll need host, port, database name, user credentials, etc.).

4. Map the fields from your form submission (for example: Name, Email, Message, Custom Field) into the corresponding columns in your MySQL table so the data lands correctly.

5. Test your Zap with a sample form submission → verify that a new row appears in your MySQL table with the correct data → once successful, turn the Zap ON. From now on, each new form submission will automatically be inserted/updated in MySQL.

With the integration of forms.app and MySQL, you can automate your data-capture workflow. Every new submission will be stored and usable in your database without manual effort.
