How to use a custom domain for your form URL
The first stage to white-labeling your online forms and surveys is to use a custom domain. In this article, you will learn how to use a custom domain for your form URLs with easy steps.
Preparing your custom domain
Before using a custom domain for your forms, you must prepare it first. Here are the steps you should follow:
1. Get your domain name and create a subdomain (for example, “quiz.yourwebsite.com”).

2. If you have your domain name ready, simply go to DNS settings and add a new CNAME record that points to forms.app.

3. Here’s how your DNS record should look like:

How to change DNS settings on popular providers
While the process is similar on all providers, you can find how to add DNS records to your domain with a quick search on the tool you use. Here are the help articles from popular providers:
Adding a custom domain to your account
Once you have completed the steps above, all you have to do is open your forms.app account and add your custom domain to your account.
Please note that you need a Premium plan to use custom domains.
1. While you are on the Dashboard, click on your profile picture and choose “Account”:

2. Click on Domain Name from the left menu and select the Custom domain tab:

3. Simply enter your domain details and hit the Apply button:

After you finish everything, your form URLs will look like this:
“quiz.yourwebsite.com/formname123”
4. Also, you have to add a TXT file to prepare your domain to use in your forms.app account. You can find the details of the TXT file in the custom domain settings section on forms.app. It is a similar process to adding a CNAME; instead of CNAME, you just need to select TXT and add the value shown in the domain settings in forms.app.

Once you set up a custom domain, it usually takes 30 minutes to 2 hours for your domain to be verified. However, this process can take up to 48 hours.
