When you use forms.app as the trigger (“New Form Submission”) and Folk as the action in Zapier, the workflow becomes: a form submission in forms.app → Zap triggers → a person or company record is created (or updated) in Folk CRM. This removes manual data entry and ensures your CRM reflects every submission.

Folk’s Zapier connector supports actions like Create Person, Create Company, Find Person, Update Person, Update Company etc.

What you’ll need

  • A forms.app account (to build and publish your form and collect submissions)
     
  • A Zapier account (to build the automation workflow)
     
  • A Folk CRM account (with necessary permissions to create or update people & companies)
     

5 Steps to Set Up Folk + forms.app Integration
 

  1. In your forms.app dashboard, build and publish your form so submissions begin arriving.
     
  2. In Zapier, click “Make a Zap” → choose forms.app as the trigger app → select the “New Form Submission” trigger and connect your forms.app account.
     
  3. Add the next step: select Folk CRM as the action app → choose an action such as “Create Person” (or “Create Company”, etc) and connect your Folk account (enter your API key or authenticate).
     
  4. Map the fields from your form submission (for example: Name, Email, Company, Custom answer) into the corresponding Folk CRM fields so that the data lands correctly in your CRM.
     
  5. Test your Zap with a sample form submission → check in Folk CRM that the person/company appears or is updated as expected → once successful, turn the Zap ON. From then on, every new form submission will automatically create/update the record in Folk.
     

With the integration of forms.app and Folk CRM, you can automate your lead capture and CRM workflow.
