CCPA Compliance

CALIFORNIA PRIVACY RIGHTS

California consumers have a right to knowledge, access, and deletion of their personal information under the California Consumer Privacy Act(CCPA). California consumers also have a right to opt out of the sale of their personal information by a business and a right not to be discriminated against for exercising their California privacy rights. forms.app does not sell the personal information of California consumers and does not discriminate in response to privacy rights requests. 

forms.app provides notice of our privacy practices in our Privacy Policy. The Privacy Policy includes what personal information is collected, the source of the personal information, and the purposes of use, as well as whether forms.app discloses that personal information and if so, the categories of third parties to whom it is disclosed. Unless otherwise expressly stated, all terms in these CCPA Notice have the same meaning as defined in our Privacy Policy or as otherwise defined in the CCPA.

I- If you are a California consumer, you may have the following rights:

a. Access. You may have the right to access:

  • the categories of personal information we have collected about you,
  • the sources from which that information was collected,
  • the business or commercial purpose for collecting your personal information,
  • the categories of third parties with whom we share your personal information,
  • the specific pieces of personal information we have collected about you,
  • the categories of personal information we sold about you,
  • the categories of third parties to whom we sold personal information about you(if any), 
  • the categories of personal information we disclosed for a business purpose.

b. Deletion. You may have the right, under certain circumstances, to request that we delete the personal information you have provided to us. 

c. Non-discrimination. You have the right to be free from discrimination related to your exercise of any of your California privacy rights.

d. Verification. In order to protect your personal information from unauthorized access or deletion, we may require you to verify your credentials before you can submit a rights request. If you do not have an account with us, or if we suspect that your account has suffered fraudulent or malicious activity, we may ask you to provide additional personal information for verification. For forms.app members, we may require that you confirm certain pieces of personal information that we have on file and/or log into your forms.app account. If we are subsequently unable to confirm your identity, we may refuse your rights request.

e. Right to Know. You have the right to know the categories of personal information, categories of sources, and/or categories of third parties related to the processing of your personal information. 

f. What you can do to manage your privacy. You have choices when it comes to managing the privacy of your personal information.

g. Update your privacy settings. You may update your privacy settings by visiting your account settings.

II- How to Exercise Your California Privacy Rights

California consumers can exercise their rights directly or through an authorized agent by filling out this form. If you do not have a forms.app  account, forms.app will ask you for information that we consider necessary to verify your identity for security and to prevent fraud. This information may include name, contact information, and information related to your transaction or relationship with forms.app, but the specific information requested may differ depending on the circumstances of your request for your security and to protect privacy rights. 

If you want to delete your forms.app account please fill in this form. Please note that if you choose to cancel your data and you are a customer, your account will be deleted after 7 days you request all data in your account will be permanently deleted from our systems. If we delete your personal information, we will both render certain personal information about you permanently unrecoverable and also deidentify certain personal information.

As noted above, we do not “sell” personal information as most people would typically understand that term. However, we do allow certain third-party partners and providers to collect information about consumers directly through our services for purposes of analyzing and optimizing our services and ads, providing content and ads that are more relevant, measuring statistics and the success of ad campaigns, and detecting and reporting fraud. To the extent this practice is interpreted to constitute a “sale” under the CCPA, please see our Cookie Policy for more information including how you may be able to exercise your rights to opt-out of cookies, analytics and personalized advertising.
